The Rating field asks users to score something on a fixed scale, such as the condition of an asset. This article covers how to add a Rating field, set up its scale, and configure its settings.
How the Rating field works
A Rating field shows a row of numbers, from 1 to the highest rating you choose. Users tap a number to give their rating. You can add a label for each number, so users know what it means.
For example, an inspection form can rate a unit's condition from 1 to 5. Here, 1 is Poor and 5 is Excellent. Every inspector uses the same scale, so you can compare results across sites and over time.

Common uses include:
Inspections: Rate the condition of equipment or parts to spot what needs repair
Surveys: Ask customers how satisfied they are with a service
Evaluations: Rate skills such as communication or teamwork
Add a Rating field
In the Form Builder, select Add Fields in the side bar.
Under Numeric, select Rating to add it after the selected field. You can also drag it onto the canvas where you want it.

The field appears on the canvas as Rating Field, and its field settings open on the General tab. A new Rating field uses a scale of 1 to 7, with N/A turned on.
Set up the rating scale
In Field Label, enter the question users see, for example Overall Condition.
In Rating Max, select the highest rating. You can choose up to 10.
In each Label for box, enter what that rating means, for example Poor for 1.
Turn on Show Labels to show the label next to the field after the user selects a rating.

To let users skip the rating, keep Allow N/A turned on. Users then see an NA choice before the numbers. To let users select more than one rating, turn on Allow Multi Select.
Configure the field
Use the General, Behavior, and Appearance tabs in Field Settings to control how the field works.

Select Save Draft to save your changes. Users see them after you select Publish.
Set up the field with the Assistant
You can also ask the Assistant to add a Rating field and set up its scale.
Select Assistant in the side bar.
Describe what you want, for example Add a rating field called Customer Satisfaction after Description, from 1 to 5, with labels Poor, Fair, Good, Very Good, and Excellent.
Send the message.

The Assistant can set the highest rating, add labels, allow N/A, allow multiple selections, and make the field required. Check the field settings after each change, because the Assistant may turn on Allow N/A for you.
Note
Assistant changes aren't saved until you select Save Draft.
