After Authorization, the wizard shows the Application Information step. Here you choose the Quickbase app to set up the extension in. Setup Table skips this step, because the app is already chosen; see the installation wizard overview.
The Application Information step
Under the heading Select a Quickbase Application, the step shows:
The app grid. Every app that the user token from Authorization can access, one per row. Select the radio button, or anywhere on the row, to choose an app. For example:

If the app you want isn't listed, select Previous and choose a different token, or give the token access to the app in Quickbase, then open the wizard again.
Authorize Only No Setup Required (optional). A checkbox. Check it to authorize the app for the extension without changing the app: the wizard saves the app and the user token with your Extensions account, and nothing else. Use it for a copy of an app that already has the extension's tables and fields. The checkbox appears when you opened the wizard from the Extensions Portal for an extension that has extension-specific steps.
Upgrade Setup (optional). A checkbox that appears only for XL Docs and JuicedSign. Check it to add or update the fields of an existing setup without doing a new one. The app must already have that extension's tables. For XL Docs, see Wizard upgrade.
You can check only one of the two checkboxes; checking one makes the other unavailable. Either one makes this the last step, so the button at the bottom reads Finish instead of Next.
What Next does
Next (or Finish) stays unavailable until you choose an app. When you select it, the wizard checks that:
The user token can see the app's tables.
The token can create tables and fields in the app, if the extension adds them.
The app is ready for the extension. For example, EZ File Importer stops if the app already has its tables, and Package Builder stops if the app has no document generation extension yet. With Upgrade Setup, the app must already have the extension's tables.
If everything checks out, the wizard goes on to the extension-specific steps. When this is the last step, Finish saves the app and the user token with your Extensions account. If the app was already authorized for this extension, the token you chose replaces the one on file for that app. The finish screen then shows one of these:
